Medical Information Sheet

This is a simple sheet you can keep with you that lists the most important medical information in one place.

It includes things like diagnoses, medications, allergies, medical equipment, baseline symptoms, and emergency contacts. It gives doctors, nurses, and EMTs a quick way to understand what matters most right away.

Edit our sheet for your specific needs or build your own. We recommend keeping a copy in a bag that always goes with you, a wheelchair pouch, your car, or a medical binder. Remember to update it anytime medications or care needs change.

Emergency Go Bag

Emergencies are stressful, and it’s hard to think clearly when you’re rushing to the ER or riding in an ambulance. This checklist is meant to help take some of the pressure off.

It includes the most important things that can make a big difference once you arrive, like ID, insurance, medical information, chargers, and any supplies that help you stay safe and comfortable.

This is not meant to be perfect or one-size-fits-all. It’s a starting point you can adjust based on your needs, and it can be helpful to keep a bag packed ahead of time if emergencies happen often.
